You fall asleep without a problem. Then, at 3am, your eyes open, your heart is going, and your mind is suddenly running through everything you have to do. You are wide awake for an hour, and you drag through the next day.

If that is you, I want to take one thing off your shoulders first: you are not a bad sleeper, and this is not simply "getting older." That exact pattern — asleep fine, awake at 3am, wired — is one of the most reliable signals I see that a single marker has slipped out of rhythm. That marker is cortisol.

I look at cortisol before I change almost anything else with a client in midlife, because it sits underneath so much of what women are told to just accept: the belly weight that will not move, the afternoon crash, the anxiety that has no obvious cause, the workouts that leave you flattened for three days. This is the first in a series of guides where I walk you through the markers I actually test — one at a time, in plain language. We are starting with cortisol.

What cortisol actually is

Cortisol is a hormone your adrenal glands release on a daily rhythm. It is not a villain, even though the internet has cast it as one. You need cortisol. It is part of what wakes you up in the morning, what carries you through a hard day, and what keeps your blood sugar steady between meals.

The key word is rhythm. In a healthy pattern, cortisol is highest in the first hour after you wake — that is the surge that gets you up and moving — and then it tapers down across the day so that by night it is low and melatonin can do its job. Picture a wave that crests in the morning and settles by bedtime.

The problem is almost never that you have "too much cortisol" as a flat number. The problem is the shape of the wave. When cortisol is low in the morning when it should be high, or high at night when it should be low, or spiking at 3am when it should be flat — that is when you feel it.

Why it matters — especially in midlife

Here is what cortisol out of rhythm does to your metabolism, and why it hits harder in perimenopause and menopause.

It raises your blood sugar. One of cortisol's jobs is to mobilize energy by pushing glucose into your bloodstream. Useful if you are running from something. Not useful when it is happening on a low simmer all day because of chronic stress, under-eating, or poor sleep. Persistently elevated glucose asks for more insulin, and over time that pushes you toward insulin resistance — which makes fat loss harder and fat storage easier.

It parks fat around your middle. Cortisol preferentially drives fat storage to the abdomen, specifically the deep visceral fat around your organs. This is why so many women tell me the weight "moved to my stomach" in their forties, even when nothing else changed. It is not a willpower story. It is a hormone story.

It breaks down muscle. Cortisol is catabolic — it can break muscle down for fuel. You are already fighting to hold onto muscle as estrogen declines. A cortisol pattern stuck in overdrive quietly works against every rep you do.

It wrecks your sleep, which raises cortisol further. Poor sleep elevates cortisol, and elevated evening cortisol wrecks sleep. That is the loop the 3am wake-up lives inside.

Now layer midlife on top. Estrogen and progesterone act as buffers on your stress response. Progesterone in particular is calming — it supports the brain chemistry that helps you downshift. As both decline through the menopause transition, you lose some of that buffer. The same stress load that you shrugged off at 35 now lands harder and lingers longer. This is not weakness. It is a real, measurable change in your physiology, and it is exactly why the strategies that worked in your thirties stop working now.

The signs it is out of rhythm

You do not need a lab to suspect cortisol. These are the patterns I hear most:

  • You wake at 3–4am, wired, and struggle to fall back asleep
  • You feel "tired but wired" — exhausted but unable to switch off
  • You get an afternoon crash around 3pm, then a second wind at night
  • Weight has settled around your midsection, disproportionately
  • You feel anxious or on-edge without a clear reason
  • Salt and sugar cravings, especially late in the day
  • Hard workouts leave you drained for days instead of energized
  • You are "doing everything right" and the scale will not move

One or two of these can be a lot of things. Four or five together, and cortisol rhythm is almost always part of the picture.

Your optimal range — a pattern, not one number

This is the part most people get wrong, and it is why a single cortisol reading can be so misleading.

Because cortisol follows a daily curve, one blood draw at 9am tells you very little. Your cortisol is supposed to be different at 8am than at 8pm. What matters is the shape across the day. A healthy pattern looks like a strong morning peak, a steady decline through the afternoon, and a low, quiet evening.

That is why I do not chase a single "normal" number. I want to see the rhythm: is the morning peak there, or is it blunted? Is evening cortisol staying elevated when it should be dropping? Two women can have an identical mid-day number and completely different problems — one with a healthy curve, one with a flattened or reversed one.

What pushes it the wrong way

Cortisol rhythm gets disrupted by more than "stress" in the way we usually mean it. The common drivers I see:

  • Chronic psychological stress — the obvious one, but rarely the only one
  • Under-eating, especially too little protein and too few carbohydrates — your body reads restriction as a threat
  • Blood-sugar swings — skipped meals and high-sugar snacks create glucose crashes that trigger cortisol to rescue you
  • Over-training — too much high-intensity work with too little recovery keeps the stress response switched on
  • Poor or short sleep — even one bad night raises the next day's cortisol
  • Caffeine too late — coffee after early afternoon can keep evening cortisol propped up
  • Alcohol — it fragments sleep and disrupts the overnight cortisol pattern, which is part of why a "nightcap" backfires

Notice how many of these are things women are often praised for: eating very little, training very hard, powering through on little sleep. In midlife, with less hormonal buffer, that combination is a direct route to a broken cortisol curve.

How to bring it back

You do not fix a 3am wake-up at 3am. You fix it during the day, by rebuilding the rhythm. None of this is glamorous. All of it works.

Anchor your morning. Get bright light — ideally outdoor light — within an hour of waking. This sharpens the natural morning cortisol peak so the curve has somewhere to fall from, and it sets your clock for better sleep that night.

Eat protein early. A real breakfast with 30+ grams of protein steadies blood sugar for hours and tells your body it is fed and safe. Skipping breakfast or running on coffee alone keeps the threat signal on.

Stop under-eating. If you have been in a long, aggressive deficit, that is very likely part of your cortisol problem. Eating enough — especially enough protein and smart carbohydrates around training — is not the opposite of your goals. It is often the missing piece.

Trade some intensity for capacity. Strength training and daily walking regulate cortisol. Endless high-intensity cardio, done depleted, does the opposite. In midlife, train to be built, not to be wrecked. A daily walk is one of the most underrated cortisol tools there is — it lowers stress hormones at an intensity you can sustain for life.

Guard your evenings. Caffeine cutoff by early afternoon. A real wind-down that is not a screen. Protein and fat at dinner to hold blood sugar overnight, so you are less likely to get the 3am glucose dip that pulls cortisol up.

Protect sleep like it is the intervention — because it is. Everything above compounds through sleep. Consistent sleep and wake times do more for your cortisol curve than any supplement.

If your hormones are also shifting — and in perimenopause and menopause they are — regulating cortisol is often what makes the rest of the picture treatable. It is the floor everything else stands on.

How we test it — test, don't guess

You can do a lot with the strategies above on your own, and I want you to start. But if the pattern is entrenched, guessing gets expensive in time and frustration.

When I want to actually see a client's cortisol, I do not rely on a single random blood draw. I look at the rhythm across the day, alongside the other markers cortisol interacts with — blood sugar, thyroid, and sex hormones — because they move together. A flattened cortisol curve with a sluggish thyroid and falling estrogen is a very different plan than any one of those alone. This is the whole reason I look at the entire system before I change a single thing. You cannot fix a rhythm you have not measured.

That is the principle under everything I do: test, don't guess. Your body keeps receipts. The answers to why you feel the way you feel are usually sitting in a vial of blood, not in another round of trying harder.
